Preparing for Your First Appointment
What to Bring:
- Picture identification (driver's license, state ID, etc.
- Your social security number
- Your insurance card
- Your completed Patient Information Form, if you did not fill out the online form below
- An up-to-date list of your current medications
What to Expect:
- At your first appointment, we will review your medical history, conduct a thorough exam, and take or schedule x-rays.
- Depending on the nature of your issue, we may perform an adjustment to help provide relief. Note that after your first adjustment you may experience some pain. If an application of ice or over-the-counter medications such as Tylenol or Motrin don't help, or if you're still having pain after 48 hours, please contact our office.
